Treatment note templates are traditionally made with the intention of being clinician-facing, meaning that your patients will never see them. Oftentimes that means questions are written in shorthand or are not very "patient-friendly". 

When configuring your template, it's best to remember that at least parts of your template will be patient-facing. 

For example, instead of a question being created as, "Medications" it would be better to pose it as a question/instruction, such as "Please list any medications you are taking".
