Custom forms in Finger-Ink are built by piggybacking off of Cliniko's treatment note template feature. 

That means that if you want to create a new form or want to add extra questions to the built-in Intake form, you'll need to create a treatment note template in Cliniko first (which we'll go over).

This and the next few steps that follow in this guide will take you through an example of creating a custom consent form in this way and will be based on the following form:

By the end of this guide, you'll have a good understanding of how things work and will be well-equipped to make your own custom forms.

Let's begin!

Step 1 - Creating a new treatment note template in Cliniko.

Our first step is to create a new treatment note template in Cliniko by going to Settings > Patients > Treatment Note Templates > Add Template:

(Quick Tip: It's easiest doing this on your computer) 

Give the template a name, in this case, "Consent":

Then add a new section by tapping the "Add Section" button and give it a title. In this case, we'll call it "Consent" again (you may notice I skipped the Name question – you'll see why later):

Change the question type to Paragraph Text and give the question a title of "Procedural Consent" and include the hashtag #other-policy (more about hashtags later):

We'll now add the first bit of the consent agreement. Copy the text below and paste it into the "Default answer":

This procedure has been fully explained to me and I am aware of its benefits as well as its risks. Alternative treatments have been made clear, including no treatment at all, and I have chosen to proceed with the surgery.

I understand that this procedure involves local anaesthetic and that as with any surgery there are a number of risks including infection, adverse reactions, and/or postoperative pain.

We're now going to repeat this for the other parts of the consent agreement.

Add another question, change it to a "Paragraph Text" and call it "Cancellation Policy" with the hashtag #other-policy. Then copy and paste the following text into this question's "Default answer": 

If you wish to cancel an appointment, we require at least 24 hours notice of cancellation. If an appointment is cancelled less than 24 hours before the scheduled appointment time, you will be charged 50% of the amount due. Any missed appointments will be forfeited and any future appointments made will be charged separately.

Repeat these steps once more for the final bit of consent.

Add a new question, change it to a Paragraph Text, call it "Privacy Policy" with the hashtag #privacy-policy (psst...this is a special one), and copy and paste the text below as the "Default answer":

In order to identify and contact you, we must collect some of your personal information such as name, phone number, address, etc. We take the utmost care to ensure it is protected against misuse, loss, interference, unauthorised access, modification and disclosure. 

We're going to include one more question that asks for an answer from a patient rather than just consent.

Add a new section called "Medical", a new Multiple Choice question, and title it "Do you smoke?" with the #required hashtag. Then add "Yes" and "No" answers, like this:

Scroll down the bottom and tap "Create Template". At this point, your template should look like this:

The next step is to get this template from Cliniko into Finger-Ink, which we'll cover in step 2.
