Every form by default will create a draft treatment note in Cliniko with your patient's questions and answers. In some cases this might make sense – for example, for a medical history or some sort of medical questionnaire form. And if you're happy with the answers patient's provide, you may even want to make the note final. Other times though, it doesn't make sense. For example, there's no need to have a treatment note of your consent forms.

How to change the treatment note behaviour
When editing your form from within the online editor, at the top there's a tab for "Workflow". Click on that.
One of the options is for the treatment note behaviour. Modifying these settings and saving and publishing your form will change the treatment note behaviour of your form once completed by a patient.
To turn off the treatment note – toggle the switch off
To make the treatment note a "draft" – toggle the switch on and choose "Create as draft only"
To make the treatment note "final" – toggle the switch on and choose "Create as finalised"
