Your logo and brand comes through beautifully with a custom Theme.
Only the right questions are asked of the right people at the right time (taking advantage of visibility logic).
Your Forms are leveraging any custom patient fields you might have appropriately.

How to use the Form Conversion Service
Once you've created a Finger-Ink account:
Send us your Forms as PDF or Word docs — either to support@finger-ink.com or using the chat bubble in the bottom right-hand corner of the screen.
We'll take a look and send you a quote for the Service.
Once you accept the quote and make payment, we’ll get to work.
There might be a bit of back-and-forth with questions, after which time your Forms are loaded to your account.
You test out the Forms and let us know of any changes required.
Either you're thrilled with the Forms, or you're not and we'll remove the Forms from your account and refund your payment in full.
Email your Forms to us at support@finger-ink.com, or send them to us via the chat bubble in the bottom right-hand corner of the screen.
How to use the Form Review Service
Once signed into the Portal:
Use the chat bubble in the bottom right-hand corner of the screen to let us know you'd like a Form Review.
We'll take a look at your existing Forms and send you a quote for the Service.
Once you accept the quote and make payment, we’ll get to work.
There might be a bit of back-and-forth with questions, after which time we'll make duplicates of your existing Forms and get to work.
You test out the Forms and let us know of any changes required.
Either you're thrilled with the Forms, or you're not and we'll remove the Forms from your account and refund your payment in full.
