How do I add a body chart, drawing or image field?

A picture is worth a thousand words, so get your patients to draw you one!

We've got a few field types that allow you to either gather, or present information in a visual way. These are the body chart, drawing and image field types.

A quick primer

Before diving into how to add one of these fields, here's a quick refresher on each type.

Body chart

The Body chart field type is exactly what you need when you're wanting the patient to indicate where they're experiencing pain. You choose the background image (i.e. the actual chart to use) in the Form Editor, then this is presented to the patient as they're filling out the Form.

☝️ The Body chart is only produced on the PDF if the patient draws something on top of the chart. If the field is set to required, then the patient must draw something.

Drawing

The Drawing field type works similarly to the Body chart field type, except that there's no background image.

☝️ Like the a Body chart field, a Drawing field is produced on the PDF if the patient draws something. If the field is set to required, then the patient must draw something.

Image

The Image field type works in the same way as the Body chart field type, except that the patient cannot draw on it, and it's always produced on the PDF.

It's useful for things like info-graphics — where you're wanting to convey a lot of information in a compact manner.

Adding these to your Form

The first step when adding any field is to select the Add field button on the Form itself. This adds a new single-line text field. The next step is to change the field type.

Adding a drawing field

The drawing field is the most simple of the three to add — because it doesn't require uploading an image. Just select the field type from the drop down selector, and select the tick to confirm. Done!

Adding a body chart or image field

Both the body chart and image field types require an image to be uploaded as a background. In the video below, I change to the body chart field type, then select the browse button to select an image from my computer. Finally, I publish the Form. This uploads the image and makes my changes public.

☝️ In the video above, I fast-forwarded the part where I selected an image on my computer — it's not usually that lightning quick!

Adding an image field is almost exactly the same workflow, so I won't go into the details.

The PDF & Treatment Note

Images always display on the PDF, and are always linked to from the Treatment Note.

Body charts and drawings, on the other hand, only appear on the PDF and Treatment Note when the patient has made an annotation. Put another way, if the patient skips drawing on either field, they won't be reproduced on the PDF or linked to from the Treatment Note.

Here's how the Treatment Note looks with both a body chart and drawing:
