Let's take a look at the different parts of a Screen prompt and how you can modify them. A form is made up of sections, screens, and fields. Each form must have at least one section, each section must contain one or more screens, and each screen must have one or more fields.
Now, let's focus on the various components of a screen that you can change. These include: Screen Prompt, Screen Sub-Prompt, Skip Button Text, Field Prompt, and Field Placeholder Text.
Changing Text at the Screen Level (Prompt, Sub-Prompt, Skip Button)
A question is called a "Screen," which includes a question prompt, sub-prompt, fields, and a skip button. To modify the screen prompt:
Screen Prompt:
Click on the screen prompt text and edit as needed.
Sub-Prompt:
Enable the sub-prompt if necessary by clicking on "Show sub-prompt" in the settings.
Change the sub-prompt text similarly.
Skip Button:
Manually change the skip button text from "skip" to your preferred text.
Changing Text at the Field Level (Field Title, Placeholder)
Each screen contains (at least) one or more fields where patients input their answers.
To modify Field Prompts:
Click on the field text and edit as desired.
To modify the field's placeholder:
Click on the placeholder text (e.g., "Example answer") and edit it accordingly.
Remember, only field types allowing text entry will display a placeholder. Check the different field types here for more information.
Please make sure to save your changes and publish them.
