Over time, you’ve probably created and customised your own forms to suit the way your clinic works — tweaking questions, wording and workflow as you’ve learned what gets the best results.
The shared forms library exists for two reasons:
First, to give you inspiration — showing what’s possible with some of Finger-Ink’s more advanced features, like conditional logic, conditional completion flows, multi-page layouts, scoring and more.
Second, to give you a shortcut to common PROMs forms. We’ve pre-built a number of clinically-validated PROMs so you don’t have to start from scratch.
How it works
The first step to using the shared forms library is just to visit it — you'll find it tucked away in the Portal, under the "Explore" heading:
Once you're there, you can click on a form to get a preview of that form. That will show the form in the familiar "editor", but you won't be able to change it. You can, however, use either of the copy buttons to add it to your account for use with your own patients:
Once copied, the form is yours. You can customise it further as you see fit — changing the theme, adding or removing questions, and changing the completion flow.
Searching
We add tags to each form we share, to make it easier for you to find what you're looking for. You can enter your own text in the search box, or just click on a tag to find all the forms that match:
Sharing your own forms
If you've got a form you're particularly proud of in the way that you've structured it, or you'd like to give others inspiration, you can submit your own forms for sharing via the shared forms library. Just tick the form you'd like to share and use the drop-down menu to share:
That will bring up the share box — so just tell us what's special about the form and submit it for approval:
You'll be notified via email when you've shared a form, and whether the form has been approved or rejected.
If it's rejected, please don't dispair! We may just have another very similar form in the shared forms library already.
