Tap on "Add form" to add a new form. As the example form we're going to use includes both intake-related questions and policies, go ahead and choose "New intake & consent form"

What this does is create a new form for you with a few questions already added to give you a head start (don't worry, we'll show you how to add, modify, or remove these later). At this point the form lives only within your account portal online in an "unpublished" state, meaning it doesn't yet exist on your iPad. We'll explain more about that later.
