This is Step 3 of our step-by-step guide on creating your very own Intake form

This is an optional step, but you'll probably want to do this. The very first thing your patients will see when filling out a form is a "welcome screen" that has a background image and a little bit of text to introduce the form (e.g. what form they're filling out, why, etc)

Change the welcome text:

You can change both the "Hello" text and the introductory paragraph. To change either of these:

  • Tap on the text you want to change

  • Edit the text as you wish

Change the background image:

You can also change the background image to something more on brand or suitable to your clinic offerings or location.

If you changed any of the text, you may have noticed a configuration box pop up on the right-hand side.

This box is the configuration box for whatever item is active, whether that's the form itself, a screen, a particular field, etc. We'll go over this configuration some more later, but for now you can use this box to upload a new image for the background. Go ahead and do that now.

Save your changes

After each little chunk of work you do, it's a good idea to save your changes by tapping "save draft" in the top right-hand corner.
