Where do filled Forms go in Cliniko?

A filled Finger-Ink Form is processed through a series of pre-defined steps.

All filled Forms are first recorded as a "Response" in our Portal. As far as where they're stored in Cliniko, that's mostly up to you. You specify where it should go on the Form template, and can override this manually if you like.

By default three things will happen.

  1. The patient record is updated in Cliniko

  2. A PDF will be uploaded to the Files section on the patient record.

  3. A draft treatment note will be created.

  4. (iPad app Forms only) The patient is marked as arrived on the appointment.

The patient record is updated in Cliniko

The first thing that happens after a Finger-Ink Form is filled is the updating of the patient record. Have a look:

Almost any field on the patient record can be updated through a Finger-Ink Form. That includes all the "difficult to handle" ones that might need the patient to select from a list (like "title"). Unless they don't want to. Then they can specify their own unique answer — just like when editing the record in Cliniko. It also includes your Referral Sources (handy for your reporting later on), and custom patient fields.

A PDF is uploaded to Files

The second thing that happens after a Finger-Ink Form is filled is the uploading of a PDF version of the Form to the Files section on the patient record.

A draft Treatment Note is created

The third thing that happens when a Finger-Ink Form is filled is the creation of a Treatment Note against the patient record in Cliniko.

This is configured to be a draft Note by default, but it can be created as finalised as well. Also by default:

  1. The Note is created as the practitioner the patient is seeing — so only they can edit, finalise or archive it.

  2. The note contains only answers that are not part of the patient record — so name, phone, custom fields etc. aren't included.

One of the cool things about the Note is that we can define hidden sections on the Form itself, which won't get shown to the patient, but will come through to the Note. This is perfect for a canonical medical history note, pinned to the top of the Treatment Notes on the patient record.

Marking the patient as arrived

You may already know that we've got a Check-in system. This works by marking a patient as arrived in Cliniko when that patient selects their appointment. Finger-Ink Forms filled out on the iPad also perform this action — marking the patient as arrived in Cliniko.

☝️ Forms filled via the web do not mark the patient as arrived. Only Forms filled via the iPad app, in the clinic, do this.
