From the appointments screen

When your patient or client comes in for her/his first appointment, chances are you'll need to collect additional information that wasn't collected at the time of making a booking.

Here's how you do it:

  1. Navigate to the Appointments section of the app (the watch icon at the top of the left-hand menu)

  2. Find and tap on your patient's name/appointment.

  3. Select the appropriate forms, then tap start forms.

At this point, the first chosen form will start specifically for the chosen patient. Any information that was known at the time of making a booking will be pre-filled (such as name, email address, etc). When that form has been filled and signed, the next form (if any) will display — and so on and so on until there are no more forms to fill. The iPad will then ask the patient to take the iPad back to reception.

Once the form is completed, the patient's record in the practice management system will be updated with any new information, a signed PDF will be attached to their Files section, and a treatment note will be created (if specified by the form workflow).
